The aircraft left RAF Mildenhall at 10.55 on Ops to Bocholt, but crashed 2 minutes after take off with an engine on fire. It blew up over Mundford, Norfolk, 4 miles NE of Brandon. All the crew were killed and are buried in a collective grave at Cambridge City Cemetery. This was the last Lancaster of 15 Sqn to be lost in the war.

Sgt Matthews had lived at Shanklin, Isle of Wight.
